Diamond HD 2900XT cuts out

I have a diamond radeon hd 2900xt in my computer and when i play games like call of duty 4 sooner or later the screen will just cout out. When it cuts out it will usually say like no signal then sometimes input d sub, and sometimes it recovers from it and other times it just freezes forcing me to restart my computer manually. I've told it to run at 100% with the ati tool and it still cut out at some point in time, then i said to keep the temperature at 60 and well it seemed fine but it still cut out.

My computer specs are:
Motherboard: GIGABYTE GA-M57SLI-S4 socket AM2
CPU: AMD Athlon 64 X2 5600+ 2.8GHz
RAM: 4gb (1gb x 4) of OCZ ati crossfire certified ddr2 800mhz
HDD: Seagate 250gb 7200rpm sata 16mb cache
CPU fan: Zalmon CNPS 9700 110mm
Case: Cooler Master CAC-T05 WW( ww just means it has a side window) with one 80mm fan in the front and one 120mm fan in the back
PSU: OCZ StealthXStream 600w
Other: Apevia 80mm case fan

Right now my computer is near my vent so that when the cool are comes out the 80mm front fan can suck it into the computer, and i usually keep it closed when warm are is coming out of it.

Also, i ran a memtest overnight and there were no errors found.
