New 6200 PCI, artifacts/weird lines?

Hello, all...

I just recently bought a 3d fuzion 6200 PCI, installed it and the corresponding drivers included on the CD (version 84.21). I believe I've done everything correctly, but there are stand dashes appearing on certain windows, mostly install windows, my AIM BL, or anything "ms window" window... don't know how else to describe it.

I previously had a evga fx5200 and this problem never presented itself, so I was wondering if it was a defective GPU, or anything? Any help would be appreciated, thanks in advance!

Dimension 2350
P4 2.2ghz
768mb RAM (256 PC2300, 512 PC2100)

Looks like a wonky card, if you can get it replaced I'd try that first
