Petition on a website?

Is there anyway to include a petition into a webpage, but so that the 'signatures' don't have to be moderated - they're just automatically added to the bottom of the list?

Sure, there's third-party websites that have petitions on but I want to encorporate the petition into my own page.

For a start, you need "clever" webspace, with script capability, not bog-standard ISP or FWP space.

Found some for ASP - eg.
